Steps to Referring Your Patient to Dr. Mark Lavin

  • Please email (contact.us@marktlavindds.com) or fax (605.332.6305) a copy of our Referral Form to our office

  • Send us any current films that correspond with the site being evaluated

  • Provide any special considerations regarding the patient

  • Make sure the patient brings his or her insurance card to the initial appointment

  • Any relevant images that can be shared from your practice help to minimize the cost of the initial exam
